This will give this main caregivers consistent days off for themselves EVERY WEEK (not just respite care) to care for their own needs.

This essential document acts as a roadmap, detailing the medical, emotional, and physical needs of the person requiring care. But what sets an effective plan apart is its inclusion of the caregiver’s needs.
A robust plan of care should:
- Highlight the care recipient’s ailments, medications, and devices.
- Provide step-by-step caregiving instructions.
- Outline strategies to support the caregiver, such as scheduling consistent additional caregivers, who work 2-3 days a week.

What is a Plan of Care?

A Plan of Care is a Book of Caregiving Instructions or manual, filled with all information pertaining to the individuals ailments and care needs, so everyone involved with their care will know what to do and how to provide quality caregiving. Most professional caregiving agencies refer to this as an ISP, Individual Support Plan.
However the SHS&S Organization refers to their plan of care as an HSP- Holistic Support Plan. This is because since our primary focus is to support and care for the need of the caregiver before the person needing care so they can have the strength and resources to care for this care recipient, our (Manual ) is filled with information about the person needing care ( ISP) like professional agencies, but also information and training to support the main caregiver.
Why is it Imperative that EVERY CAREGIVER has one in place before starting caregiving?
It’s understandable that no one would attempt to bake a pound cake for the first time without a recipe because the cake may not turn out well….Neither would a person attempt to change the alternator on their vehicle without a manual….
Any task we take on in life that we are unfamiliar with requires instructions and a Step by Step guide, otherwise the outcome can be disastrous.
Yet unfortunately, this is the sad situation many caregivers find themselves forced into, despite how serious and life threatening it could be for both, the individual needing care as well as the person providing it,. And as they begin their caregiving journey, many become overwhelmed as they navigate through all the new medications, devices, equipment, doctors orders and behaviors of the individual needing care.
So… what does the caregiver do? Who do they turn to for advice and how do they put all the pieces together and learn to live with this NEW NORM?
