Why this training exists
Caregiver burnout and professional burnout mirror each other
SHS&S was built to support family caregivers. As we kept searching for caregivers wherever they could be found, that search led us into hospitals, rehab centers, and home health agencies — and we discovered that many healthcare professionals are experiencing the same burnout as the families they serve. Caregivers and healthcare professionals are part of the same ecosystem.
Supports discharge planning standards
Helps social workers and case managers meet ACMA and CCMC standards through education, resources, and support that promote safe, effective care transitions.
Reduces caregiver overwhelm
We help families build a Personalized Plan of Care — similar to those used daily by professional caregiving agencies — filled with everything everyone involved needs to know to provide quality care.
Improves continuity of care
A stabilized caregiver follows discharge instructions more reliably, strengthening the whole care ecosystem and helping reduce complicated, avoidable readmissions.
Extends support beyond discharge
Our 4-week stabilization program reinforces education and answers questions, helping new and established caregivers alike make a successful transition into the caregiving role.
For agencies
Burnout prevention systems for care teams
In response to growing demand, SHS&S also offers a structured, agency-based implementation model — built to work within your existing workflow, not replace it.
What it does
- Identifies burnout triggers early.
- Helps healthcare professionals create a Buddy System for mutual support.
- Encourages balanced patient care and self-care.
- Strengthens teamwork, agency efficiency, and reduces staff fatigue and turnover risk.
Structured, measurable outcomes
Our systems collect structured data so care professionals and agencies can track engagement and progress. That data rolls up into monthly, quarterly, and annual reports — tangible evidence of stabilization, for the care professional, the family and caregiver, and the agency.
